Actuality: one of the first places in maternal mortality structure is occupied by puerperal- septic inflammatory disease, being situated after hemorrhages and hypertensive states. Statistics of developed countries with good quality of health care do not show maternal mortality by infection or puerperal-septic infection (Willson, 1987). Maternal mortality due to septic complications was analyzed by researchers from our country during the 19902005 period and the incidence was ranged between 12.3 to 18.1%.The morbidity structure in puerperal – septic inflammatory disease has changed in the last years, due to increased rate of cesarean surgery, microbial virulence and aggression, and antimicrobial resistance. As a result, a rapid diagnostic, conservative and effective treatment method and prompt surgical treatment are required. The aim of the study: to study the particularieties of clinical evolution of puerperal - septic inflammatory disease and to optimize diagnostic and treatment methods. Materials and Methods: a retrospective study was performed in 2017. It includes 301 postpartum females with puerperal-septic inflammatory disease, who were hospitalized in Septic Gynecology Department from IMSP SCM No. 1. Results and conclusions: by assessment it was determined that 73% of postpartum females included in this study had obstetrical - gynecological and extra genital pathologies during pregnancy. It may explain the puerperal – septic inflammatory disease association in this patients group. According to data results of this study, a severe form of puerperal – septic inflammatory disease was recorded in 11 (3,6%) of cases and radical surgery was performed. Placental retention based endometritisprevailed. The research results determined laparoscopy as the early and timely diagnosis methodthat permitted in 26 (6,6%) of cases prompt decisions concerning the surgery volume.In postpartum pelvioperitonitis, organomenajant treatment with the preservation of the uterus and of the reproductive function in the future was possible in 22 cases (7.3%), due to studying the laparoscopic approach and the use of the transabdominal drainagetechnique. According to the statistics, it is gladdening that the puerperal – septic disease incidence decresead in the last years, being rarely encountered the severe form. Due to laparoscopy as a early method of diagnosis, a surgery with uterus preservation is possible |
