Background. Bronchial obstruction is one of the most frequent clinical manifestations in childhood. However, in children under 5 years of age is rather diffi cult to establish the etiology and treatment for this syndrome. Material and methods. The study included 244 children with bronchial obstruction, including 150 children with acute respiratory infections, hospitalized to specialized units of the Institute for Maternal and Child Healthcare the Pulmonology Unit, and 75 patients with bronchial asthma. For data collection purpose a special questionnaire was elaborated. Statistical processing of the material was performed by applying statistical software SPSSv6.0. Results. Results of the case-control study revealed different clinical phenotypes of bronchial obstruction in young children: patients with bronchial obstruction of infectious etiology most frequently reported mixed dyspnoea (p<0.0001), pneumonia and bronchiolitis (p <0.0001), fever (p<0.05), sleepiness (p<0.05), and psychomotor agitation (p<0.05). Children with atopy and bronchial obstruction manifest mainly expiratory dyspnea (p<0.0001) and nocturnal dry cough (p<0.0001), without developing bronchiolitis. Conclusions. The study results revealed two clinical phenotypes of bronchial obstruction in children under 5 years of age; this will make possible an individualized approach in treating children with wheezing. |
