Background: Efficient treatment of infertility by in vitro fertilization (IVF) is usually estimated by number of pregnancies per number of patients. Analysis of evolutionary peculiarities of such cases has shown a high incidence of complications and reproductive losses.
Material and methods: A retrospective and prospective study of 991 pregnancies (2214 cycles of stimulation) occurred after IVF treatment had been carried out in 2004- 2009 at CNSRsiGM. Age of pregnant women varied between 22 and 42 years, duration of infertility - from 2 to 20 years.
Results: It was found that from 991 induced pregnancies 572 (57.7%) were monofetal and 419 (42.3%) – multiple. Miscarriage in I trimester happened in 10.21% (101pregnant women). In the second trimester 53 (5,3%) pregnancies, including 35 multiple (at term of 17-19 weeks) and 18 monofetal (at term of 19-21 weeks) ended miscarriage, caused by cervical incompetence.
Preterm delivery at 31-36 weeks occurred in 148 (18,87%) cases - 140 were multiple and 8 monofetal, delivery at term – 636 cases (81,13%). The incidence of caesarean section was 67.95%. “Take home baby” index was 81.89%.
Conclusions: It was found that IVF leads to increased number of multiple pregnancies and also to reproductive losses and caesarean sections. Rationally provided health care by fertility specialist and obstetrician can improve the final result of infertility treatment by IVF. |
