Background. Anorectal malformations (ARM) include a wide range of congenital anomalies, involving the anus and rectum. According to worldwide statistics, ARM in 40-70% are found with non-random coexistence of various congenital malformation, which influences the curative management and long-term results. Objective of the study. Estimate the impact of malformative associations on postoperative morbidity and mortality in children with ARM. Material and Methods. Based on the medical databases of children diagnosed and treated with ARM in our clinic in 2018-2020, was performed retrospective study. Results. Out of 36 children in the study group, in 61.1% of cases were confirmed associated malformations (AM). Associations with cardiovascular malformations (16.6%), urogenital (13.8%), digestive tract (5.5%), osteo-nervous (11.1%). In their nosological structure, we mention VACTERL association in 22.2%, Townes-Brocks syndrome - 2.7%, OEIS syndrome - 5.5%, Split Notochord syndrome - 2.7%. The presence of AM negatively affected the complexity and risks of radical surgical treatment. Postoperative lethality accounted for 8.1%, caused not by ARM as by the impact of AM, prematurity and perinatal hypoxic trauma. In the group of survival children, the postoperative evolution was uneven. In children with isolated form of ARM, postoperative treatment evolved with a good recovery score. In ARM with AM, the postoperative period was complicated by repeated operations, the persistence of pelvic functional disabilities and restrictions imposed by the AM in deployment of the optimal recovery plan. Conclusion. AM in MAR negatively influences the postoperative period, increasing the duration of functional rehabilitation and distancing the social integration of these children. |
