Background. Taking over the functions neorectum in children with Hirschsprung disease (HD), in the postoperative period, leads to the appearance of functional disabilities manifested by changes of the frequency of defecation, volume of the fecal bowl and fecal continence, which influences patients’ quality of life and social integration. Objective of the study. Evaluation of electromanometry (EMM) informativeness in causal differentiation of functional disabilities in children operated for HD. Material and Methods. The study included 52 children, who had persistent functional disabilities in the postoperative period. Results. In the study group, EMM of the anal canal, rectal ampulla and colon, offered us extensive information about biomechanics of the defecation, colonic motility, allowed the global assessment of the visceral-sphincterian function and the functional quantification of each muscular and neurological component involved in the pathology of transit and intestinal evacuation. The persistence of functional disabilities is directly correlated with the restoration of the rectosphincterian reflex (RRS). Staged EMM revealed the gradual recovery of RRS from the 6th postoperative month. At the same time, absence of the RRS was registered in all cases of persistence of functional disabilities, being an indicator of infrarectal obstruction that induce the constipation persistence and feces doe to overfilling. Conclusion. EMM enable objectification of the development mechanisms of functional disabilities, being useful in modeling the curative conduct of the rehabilitation process. |
