Background. The incidence of multiple pregnancy has increased significantly in recent decades, being correlated with two main factors: the increasing maternal age at the time of conception and increasing use of assisted human reproduction techniques. Objective of the study. Analysis of clinical, diagnostic and management aspects in patients with multiple pregnancy. Material and Methods. We analyzed 50 cases of multiple pregnancy out of a total of 6469 births registered during 2021 in the Obstetrics Ward 1 and 2 of the Perinatological Center of Gheorghe Paladi municipal clinical hospital. Anamnestic, clinical and paraclinical data as well as the evolution of the birth and the neonatal results were taken from the medical records. Results. Multiple pregnancy is a topical issue due to frequent maternal and fetal complications. In 34% of cases, was determined monochoreal-biamnionic pregnancy and in 66% - bicorialbiamnionic pregnancy. Pregnancy occurred naturally in 84% of cases and in 16% of cases through IVF. In 42% of cases, the twin pregnancy was registered in primiparous, in 38% - in secondary and in 20% - in multiparous. Analyzing the evolution of the pregnancy, we established that the most common complications were: term PROM (24%), imminent premature birth (22%), imminent miscarriage (18%), gestational hypertension (18%), IUGR (10%). Group data analysis shows the 100% survival of the girls. In 14% of cases, circular umbilical cord was determined in first fetus and 24%- in the second fetus. Cesarean birth occurred in 58% of cases, and 42% - naturally. Conclusion. Due to the increased risk of complications, pregnant women with twin pregnancy require closer monitoring and closer contact with medical staff (family doctor and obstetrician) during pregnancy. This involves several antenatal visits and ultrasound examinations in order to improve perinatal indications. |
