Background. Transforaminal percutaneous endoscopic discectomy (PETD) - an innovative method in the treatment of lumbar disc herniation from lateral approach. PETD performed under local anesthesia, minimally invasive incision, and endoscopic visualization allows safe removal and early rehabilitation. Objective of the study. Analysis of the efficiency surgical treatment in patients with lumbar disc herniation using the PETD method. Material and Methods. Prospective study of 35 patients with different localization of lumbar disc herniation: level L5-16, level L4-17, level L3-2; central herniation 6 (17.1%), paramedian 15 (42.85%), intraforaminal 12 (34.35%), extraforaminal 2 (5.7%) using the exclusion criteria. Follow-up at 1, 3, 6, 12 months according to the VAS and MacNab scales after performing the DEPT intervention. Results. The research analysis shows 29 cases (82.9%) excellent, 5 (14.28%) good, 1 (2.83%) satisfactory (Macnab score). Preoperative mean VAS 6.71±1.52. Postoperative mean VAS 3.1±1.3 and at last rebound 2.2±1.3 (p < 0.0001). Complications: recurrence 2 (5.63%); no cases of conversion, infection or discitis. Minimally invasive stages with removal of the hernia by endoscope under local anesthesia quantified operating time 60±4.8 min, intraoperative hemorrhage 9±1.2 ml, average verticalization 2.5±0.6 hours; discharge on the same day or the 2nd day, usual activity at 7-10 days. DEPT can be used in various pathologies with stabilization instruments. Conclusion. Transforaminal percutaneous endoscopic discectomy (PETD) is an alternative method to open surgery with multiple benefits. PETD allows safe and complete removal of disc herniation by endoscopic visualization. |
