Introduction. The role of different biomarkers in predicting the evolution and the results of treatment of acute limb ischemia (ALI) is insufficiently studied. Laboratory indications that reflect the severity of the systemic inflammatory response may be useful in estimating the risk of amputation and death. Objectives. The aim of the study was to analyze the predictive value of the neutrophil-lymphocyte ratio (NLR) in predicting mortality and the risk of major amputation in patients with ALI. Methods. All consecutive patients with ALI hospitalized in the Institute of Emergency Medicine and undergoing urgent revascularization treatment between 2019 and 2022 were prospectively enrolled. The NLR indicator was derived from the general blood test, taken preoperatively. The mortality rate and major amputations were assessed during hospitalization, at one month. Results. A total of 157 patients were included in the study. The median age of the patients was 71 years. Males prevailed - 97 (61.8%) male patients. Grade of ALI according to the Rutherford classification: grade I - 10 (6.3%) cases, grade IIA - 57 (36.3%) cases, grade IIB - 90 (57.3%) cases. In the general group the median NLR was 6.9 (25-75% IQR 5.3-7.6). The mean value of NLR was significantly higher in patients with IAE grade IIB - 7.7 (95% CI 7.9-9.9) vs. grade IIA - 5.0 (95% CI 4.2-6.4), p <0.0001. At 30 days, the mortality rate was 17.8% and the amputation rate was 10.1%.The area under the ROC curve for NLR was 0.713, and the optimal predictive value was 5.8. Conclusions: The NLR index is an informative, simple, and widely available test that can contribute to reasoned tactical decisions in the treatment of patients with ALI. |
